*Dorset Annual Health Checks Project*

Parent Carers Together (the forum for parents and carers living in BCP) and Dorset Parent Carer Council (the forum for parents and carers living in Dorset) are working together with Dorset Clinical Commissioning Group to improve Annual Health Checks locally for young people aged 14-25 years old.

The aim is to improve the health inequalities for young people with a learning disability and/or autism living in the county of Dorset in line with the NHS Long Term Plan (published January 2019).

This work will be starting very soon, and we are looking for parents and carers of young people aged 14-25 years old.

Parent Carers Together (PCT) and Dorset Parent Carer Council (DPCC) have written a joint statement to give the background to the forums, to explain our role, and how we provide the parent and carer 'voice' for the SEND community both locally and nationally.
